Discharge Summary Program

The project was spurred by local government and provincial health quality mandates requiring Toronto-area hospitals to standardize their discharge summaries. The goal was to make each patient’s hospital discharge a safe and timely transition back to their community. UHN launched a three-year, CA$955,000 project to improve the quality and timely completion and delivery of its discharge summaries. The project closed under budget and on time. Today, ninety-four percent of UHN patients now receive a timely discharge summary.
