GAC Annual Report

Annual Report Templates

During a program’s GAC accreditation period, an online report must be submitted by 31 October each year. This report includes demographic information about the accredited program, such as current enrollment, graduates, number of faculty, and more. Additionally, the report should detail any minor changes or improvements in the accredited program(s) or the institution where it is housed. All GAC-accredited programs are required to complete the Annual Report, regardless of their current accreditation status. This includes programs in the reaffirmation process as well as those recently affirmed/reaffirmed.

Use the templates below to collect the required data. Programs will not submit these templates directly but will transfer the data into GAC's Annual Report Portal (Weave).

Weave directions and training videos are available at GAC Accreditation Portal Instructions and Directions
